Hypertension portale chez un malade porteur d'une hépatite C, révélatrice d'une fistule artérioportale iatrogène - 01/01/01

Un homme de 67 ans porteur d'une hépatite C post-transfusionnelle a été hospitalisé pour hématémèse par rupture de varice oesophagienne. L'hypertension portale, n'était pas due à l'évolution cirrhotique de l'hépatite C, mais à une fistule entre une artère issue de la branche droite de l'artère hépatique et une branche droite de la veine porte. La fistule, située à la face inférieure du foie droit, et les segments hépatiques atrophiques adjacents, ont été réséqués par une sectoriectomie latérale droite (bisegmentectomie VI-VII). Les suites opératoires ont été simples. À l'examen de la pièce opératoire, il n'y avait pas de cirrhose mais une hépatite active. Cette fistule artérioportale semble d'origine iatrogène. En effet, ce malade avait eu 16 ans auparavant une gastrectomie totale pour cancer suivie d'une reprise chirurgicale pour hémorragie grave ayant nécessité une transfusion abondante responsable de la transmission de l'hépatite C.

A 67-year-old man with anti-HCV positive serum, was admitted for hematemesis by variceal bleeding. Portal hypertension, which initially was thought to be caused by a post-hepatitis C cirrhosis, was due to a fistula between a right hepatic artery and a right branch of the portal vein. The fistula located under the right liver and the adjacent atrophic hepatic segments, were resected by a bi-segmentectomy VI-VII. The postoperative course was simple. The pathological study of the resected liver showed no cirrhosis but active hepatitis. This arterioportal fistula was probably iatrogenic. Sixteen years before, this patient had undergone a total gastrectomy for cancer, followed by a serious haemorrhage requiring a massive transfusion, which was responsible for the transmission of hepatitis C.
