L’ostéopathie liée au méthotrexate a été initialement décrite à dose oncologique dans les années 1970. Si de nombreux cas ont ensuite été rapportés à dose rhumatologique, l’imputabilité du méthotrexate reste discutée dans le développement des fractures/fissures. Même si la population atteinte présente des facteurs de risque, souvent nombreux de fragilité osseuse (corticothérapie, rhumatisme inflammatoire, ménopause…), certaines caractéristiques doivent faire évoquer ce diagnostic : l’atteinte des membres inférieurs (os du pied, métaphyse tibiale proximale ou distale, fémorale distale), l’aspect en imagerie (IRM ou scintigraphie) de fissure transversale, l’atteinte multiple et récidivante au cours du temps. Le traitement par méthotrexate doit être arrêté (si le contexte inflammatoire le permet) avec classiquement une bonne cicatrisation osseuse et l’absence de récidives de fractures à l’arrêt.

Methotrexate-induced osteopathy was first described in the 1970s in the context of high-dose chemotherapy. Since then, numerous cases have been reported in patients with rheumatological conditions. However, the exact role of methotrexate in the development of fractures and bone fissures remains a subject of ongoing debate. Although the affected patient population typically presents with multiple risk factors for bone fragility — such as corticosteroid therapy, inflammatory rheumatism, and menopause — certain clinical features should raise suspicion for methotrexate-related bone pathology. These include the involvement of the lower limbs (particularly the foot bones, proximal or distal tibial metaphysis, and distal femoral metaphysis), imaging findings such as transverse fissures on MRI or bone scintigraphy, and the presence of multiple, recurrent fractures over time. When methotrexate-related osteopathy is suspected, discontinuation of methotrexate should be considered, provided that the underlying inflammatory condition allows for this adjustment. In many cases, cessation of the drug leads to favorable bone healing, with no recurrence of fractures following withdrawal.
