Riassunto

Les spondylarthropathies du sujet âgé ont souvent une présentation atypique. Nous avons observé 2 cas qui ont fait suspecter une algodystrophie. Les 2 hommes âgés de 62 et 75 ans avaient un important gonflement douloureux du pied apparu dans un cas dans les suites d'un traumatisme modéré. La présence d'une déminéralisation diffuse et intense et d'une hyperfixation scintigraphique diffuse, intense dès le temps vasculaire ont fait évoquer une algodystrophie et prescrire sans succès calcitonine, griséofulvine et pamidronate. L'association d'un syndrome inflammatoire important a fait suspecter une autre pathologie associée. Ces 2 patients avaient en fait une spondylarthropathie affirmée devant l'apparition d'une oligoarthrite, la présence du HLA B27, des anomalies sacro-iliaques et une évolution favorable sous anti-inflammatoires non stéroïdiens. Chez le sujet âgé un oedème du pied associé à une importante déminéralisation doit faire rechercher une spondylarthropathie lorsqu'ils s'associent à un syndrome inflammatoire.

Abstract

Atypical presentations are common when spondyloarthropathy develops in older patients. We report 2 cases initially mistaken for Reflex Sympathetic Dystrophy Syndrome (RSDS). Both patients were men, aged 62 and 75 years, respectively, with marked painful edema of a foot. One patient reported a moderate-energy trauma as the triggering event. Severe diffuse demineralization was noted on radiographs and diffuse hyperactivity on bone scans starting at the early vascular phase. These findings suggestive of RSDS led to treatment with calcitonin, griseofulvin and pamidronate, all of which were ineffective. Laboratory tests showed severe inflammation, promoting investigations for other conditions. Spondyloarthropathy was diagnosed based on oligoarthritis with sacroiliitis, presence of HLA B27 and a favorable response to nonsteroidal antiinflammatory therapy. In older patients, edema of the foot with severe demineralization and laboratory evidence of inflammation should suggest a spondyloarthropathy.
