Les auteurs rapportent un cas rare d'une double localisation à la cheville d'un ostéome ostéoïde chez une femme de 26 ans. La 1re lésion se situait au niveau métaphysaire tibial bordant l'articulation tibio-fibulaire distale, la 2e, également superficielle, siégeait sur le col du talus homolatéral. Le traitement chirurgical a consisté en un forage percutané des 2 lésions sous contrôle tomodensitométrique. Les suites opératoires ont été simples et, à 4 ans de recul, la cheville est asymptomatique. Le diagnostic d'ostéome ostéoïde au pied et à la cheville est souvent plus difficile que dans les localisations diaphysaires. La scintigraphie et la tomodensitométrie sont nécessaires et suffisants pour affirmer ce diagnostic. Le traitement de l'ostéome ostéoïde a largement bénéficié des progrès de l'imagerie et des techniques de chirurgie percutanée, permettant d'obtenir d'excellents résultats et de diminuer la morbidité.
Double localization of an osteoid osteoma of the ankle: percutaneous treatment by CT-guided drilling |
A 26-year-old woman presented a rare double localization of an osteoid osteoma of the ankle. The first focus was situated in the tibial metaphysic, bordering the distal tibiofibular joint. The second focus was also superficial, located in the neck of the homolateral talus. Percutaneous drilling was performed for both localizations under computed tomographic (CT) guidance. The postoperative period was uneventful and the patient has remained symptom-free at four years follow-up. Compared with disphyseal localizations, it is often more difficult to recognize an osteoid osteoma located in the foot or ankle. Scintigraphy and CT are necessary and sufficient for diagnosis. Progress in imaging techniques has greatly facilitated the treatment of osteoid osteomas, allowing excellent outcome with limited morbidity.
