Prévalence des situations palliatives au centre hospitalier territorial de Polynésie française : étude au moyen de l’outil Pallia 10 - 06/05/17

Résumé |
Objectifs |
L’objectif était d’évaluer la prévalence des situations palliatives et situations palliatives complexes au centre hospitalier de Polynésie française, afin de réaliser un état des lieux et dimensionner des structures adaptées et actuellement inexistantes.
Matériel et méthodes |
Une étude transversale a été réalisée en mars 2014 incluant tous les patients hospitalisés en services de médecine, chirurgie ou obstétrique un jour donné. L’évaluation de la situation palliative a été réalisée par les soignants grâce à l’outil Pallia 10.
Résultats |
Parmi les 367 patients inclus, 126 (34,3 %) situations palliatives ont été identifiées dont 75 (20,4 %) situations palliatives complexes. Parmi les situations palliatives, la présence de facteurs pronostiques défavorables (57,9 %), l’existence de facteurs de vulnérabilité d’ordre psychique (57,6 %) et la présence d’une maladie rapidement évolutive (43,7 %) étaient les items les plus souvent cités. Le pôle accueillant le plus de patients en situations palliatives était la médecine (54,6 %) et notamment les services d’oncologie/hématologie (85 %), neurologie (78,9 %), d’hépato-gastro-entérologie (78,6 %) et de pneumologie (76,5 %). La prévalence des situations palliatives complexes était significativement plus élevée en médecine (38,0 %) qu’en chirurgie (11,3 %) (p<0,001).
Conclusion |
La prévalence élevée des situations palliatives et notamment des situations palliatives complexes a permis d’objectiver le besoin d’une équipe mobile de soins palliatifs en Polynésie française. L’outil Pallia 10 est par ailleurs un outil intéressant pour évaluer les prévalences des situations palliatives. Cette étude devra être reconduite dans quelques années afin de redimensionner les structures en place ou en développer de nouvelles.

Objectives |
The aim of this study was to assess the prevalence rate of patients who may need palliative care, in order to plan necessary palliative care services in French Polynesia.
Methods |
A transversal study was realized in march 2014. All patients who were hospitalized in the main public hospital of French Polynesia were included, except those hospitalized in the psychiatry department. Patients who may need palliative care were identified by health care providers, using the “Pallia 10” scale.
Results |
Among the 367 patients who were included in the study, 126 (34.3%) needed palliative care (the first item of Pallia 10 was selected) and 75 (20.4%) of whom were identified as complex (four selected items, including the first one). The most frequent items among these 126 were the presence of bad prognosis factors (57.9%), of psychological vulnerability factors (57.6%), and of a rapidly progressive disease (43.7%). Patients who needed palliative care were mainly hospitalized in departments of medicine (54.6%), especially in oncology (85.0%), neurology (78.9%), gastroenterology (78.6%), pulmonary medicine (76.5%). The prevalence of patients who needed palliative care and who were identified as complex was significantly higher in medicine departments compared to surgery departments (38.0% vs 11.3% respectively, P<0.001).
Conclusion |
The high prevalence rate of patients who needed palliative care and those who were identified as complex showed that palliative care structures are necessary in French Polynesia and especially a hospital palliative care support team. Moreover, the “Pallia 10” scale was an interesting tool to assess the prevalence rate of patients who need palliative care. This study will have to be resumed in few years in order to better size or to develop new palliative care services.
