This study was funded by the National Health and Medical Research Council of Australia, the Cooperative Research Centre for Asthma, the New South Wales Department of Health, Children's Hospital Westmead, and University of Sydney, Faculty of Medicine, Strategic Research Grant 2008. C.A. was funded by the Swedish Heart Lung Foundation and Swedish Society of Medicine.
