Les effets secondaires hématologiques des antituberculeux sont mal connus. Nous rapportons l’observation d’un patient de 75 ans traité par Rifater^® pour une tuberculose pulmonaire qui, après un mois de traitement, présente une pneumopathie franche lobaire aiguë avec neutropénie à 1 218/µl et thrombopénie à 109 000/µl justifiant l’arrêt des antituberculeux.

La reprise du Rifater^® s’accompagne d’une récidive de la thrombopénie à 6 000/µl, symptomatique, nécessitant le recours à des perfusions d’immunoglobulines par voie intraveineuse. La mise en place d’un traitement par moxifloxacine, isoniazide, pyrazinamide, et ethambutol entraîne à nouveau la récidive de la thrombopénie et fait suspecter le pyrazinamide. L’arrêt, puis le test de réintroduction de cette molécule confirment le diagnostic.

Il s’agit du troisième cas publié de thrombopénie au pyrazinamide, dont l’incidence est certainement sous-estimée du fait de l’emploi fréquent de traitements combinés

The haematological side effects of antitubercular drugs are not well known. We report the observation of a patient who received Rifater for the treatment of pulmonary tuberculosis. After one month of treatment, he developed an acute pulmonary infection, with neutropenia (1218/µl) and thrombocytopenia (109 000/µl), requiring suspension of his antitubercular drugs. After the reintroduction of he again developed thrombocytopenia (6 000/µl) associated with bleeding and required treatment with intravenous immunoglobulin. The introduction of a combination of moxifloxacin, isoniazid, pyrazinamide, and ethambutol was followed by a new relapse of the thrombocytopenia. Responsibility of pyrazinamide was then suspected and later confirmed by the evolution of platelet levels after stopping and reintroducing this antibiotic. This is the third reported case of pyrazinamide induced thrombocytopenia, whose frequency is probably underestimated because of the use of compound treatment.
