Les biothérapies ont révolutionné le pronostic des patients atteints de rhumatisme inflammatoire chronique, mais ne sont pas dépourvues d’effets indésirables sévères. Le patient doit être capable de les identifier, d’acquérir des compétences d’auto-soins et d’adhérer à son traitement. Les consultations pluridisciplinaires, comprenant une consultation pharmaceutique, pourraient améliorer la prise en charge de ces patients. Le retour sur investissement est possible grâce au recours plus fréquent aux biosimilaires et à la tarification de ces consultations par le forfait de prestation intermédiaire.

Les patients éligibles sont les patients atteints de polyarthrite rhumatoïde ou de spondyloarthrites, traités par biothérapie sous-cutanée. Les critères évalués sont les connaissances des patients sur leur biothérapie par le score Biosecure, leur adhésion au traitement par le CQR5, le nombre de passage au biosimilaire réalisé et le bilan financier des consultations. Un bilan des actions déployées pour le lien ville hôpital a été réalisé.

Deux cent quatre-vingt-quinze patients (47,4 %) ont bénéficié de la consultation multidisciplinaire. Le score moyen du questionnaire Biosecure était de 69,6/100 (connaissances modérées) et 261 patients (88,5 %) étaient hautement adhérents. 57 patients (73 %) ont accépté le passage vers le biosimilaire. 197 pharmacies d’officines ont été contactées dont la totalité pour les patients avec un passage vers le biosimilaire. La satisfaction globale des patients était de 26,9/28.

Les consultations pluridisciplinaires avec implication du pharmacien pourraient optimiser le parcours de soin et la prise en charge des patients ambulatoires traités par biothérapies sous-cutanée. D’ores et déjà les patients ont exprimé leur satisfaction pour ce parcours de soins et le retour sur investissement est positif.

Biologics (bDMARDs) have revolutionized the prognosis of patients with inflammatory arthritis, but are not without serious side effects. The patient must be able to identify them, acquire self-care abilities or skills and adhere to their treatment. Multidisciplinary consultations, including a pharmaceutical consultation could improve the care of these patients. The pharmaceutical presence make it easier to switch to a biosimilar with etended patient support thanks to the community-hospital network. The return on investment is possible thanks to the more frequent use of biosimilars and the pricing of this type of consultation by the “Forfait de Prestation Intermédiaire”.
