L’embolie pulmonaire d’origine septique chez l’enfant est un événement assez rare et représente l’une des principales complications de l’endocardite infectieuse du cœur droit. Elle doit être suspectée devant l’apparition d’une douleur thoracique en présence de végétations de la valve tricuspide et/ou de l’artère pulmonaire à l’échocardiographie. Dans cette indication, la scintigraphie pulmonaire a un double intérêt ; d’une part, elle permet le diagnostic non invasif de l’embolie pulmonaire et, d’autre part, la confirmation d’une telle complication a une forte implication thérapeutique et pronostique dans la prise en charge du patient. Nous rapportons le cas d’une fillette âgée de 6 ans qui nous a été adressée pour suspicion d’embolie pulmonaire dans un contexte d’endocardite infectieuse avec présence de végétations sur le versant pulmonaire de la bifurcation de l’artère pulmonaire. La scintigraphie pulmonaire de perfusion réalisée chez cette enfant a confirmé le diagnostic en mettant en évidence des défauts de captation systématisés, ce qui a fait discuter une éventuelle prise en charge chirurgicale de ces végétations. La conduite à tenir a été l’intensification de l’antibiothérapie dans un premier temps et l’évolution clinique a été favorable avec disparition des végétations sur l’échocardiographie de contrôle.

Children's septic pulmonary embolism in a rare event and is one of the main complications of infective endocarditis involving the right heart. It must be suspected in a chest pain and presence of vegetation on tricuspid valve and/or pulmonary artery by echocardiography. In this indication, lung scintigraphy has a double interest; it allows the non-invasive diagnosis of pulmonary embolism and confirmation of a such complication has strong prognostic and therapeutic implications in the patient management. We report the case of a 6-year-old girl who was referred for suspected pulmonary embolism in the context of infective endocarditis with vegetations of pulmonary artery bifurcation side. Lung perfusion scintigraphy performed in this child confirmed the diagnosis by showing defects wedge-shaped what made discuss a possible surgical management of these vegetations. Management was the intensification of antibiotherapy initially and clinical outcomes were favorable with vegetations disappearance on echocardiography control.
