Orthodontic displacement of mandibular third molars using the orthodontic Anchorage Spider Screw® (OASS) system - 28/02/08

For some five years, miniscrews have been the most frequently used means in orthodontics to achieve intraoral and extradental skeletal anchorage. This study presents a novel technique (Derton-Perini) using segmental archwires inserted directly into the slots of Orthodontic Anchorage Spider Screws®(OASS). Two clinical case reports will illustrate the presentation.
The first case describes the replacement of a lower second molar with a wisdom tooth presenting a mesial tip and 1 cm distant from the first molar. By exercising good control, it was possible to achieve the uprighting, impaction and mesialization of the wisdom tooth in only 12 weeks, with no undesirable impact on other teeth and without requiring patient cooperation while respecting needs involving esthetics and comfort.
The second case describes the orthodontic extrusion of a lower wisdom tooth conservation of which was severely endangered for endodontic reasons. Within 10 weeks, using forced orthodontic eruption associated with a circumferential fibrotomy, we obtained satisfactory lengthening of the clinical crown to prepare for correct prosthetic reconstruction. This was achieved without periodontal surgery (which would have been difficult at this anatomical site) and with no orthodontic involvement of other teeth.
