L’observance médicamenteuse représente un comportement dynamique, variable dans le temps et dans la forme. Les répercussions d’un défaut d’observance sont d’ordre médical (perte de bénéfices immédiats et/ou à long terme) et économiques (coûts directs et indirects). Adhérer et maintenir son engagement dans la prise en charge de ses médicaments au quotidien semblent représenter un défi pour le patient chronique. Dans ce contexte, nous proposons de décrypter l’évolution de la sémantique autour de l’observance. Cette évolution sous-tend des changements de paradigmes qui redéfinissent la façon d’aborder ce concept et ouvre de nouvelles pistes d’actions. S’appuyant sur ce nouvel angle de vue que représente l’adhésion du patient à sa thérapeutique médicamenteuse, nous détaillons les différents facteurs influençant l’observance médicamenteuse catégorisés en cinq dimensions : maladie, traitement médicamenteux, facteurs démographiques et socio-économiques, patient et/ou entourage et système de soins. L’impact de ces facteurs est décrit ; les modèles explicatifs sous-jacents explicités. La compréhension intime de la problématique d’adhésion du patient permettra d’envisager des interventions personnalisées, multifacettes en réponse à la multiplicité des facteurs impliqués.

Patient compliance is a dynamic behaviour, which varies in time and in shape. The impact of poor adherence is medical (loss of immediate benefits and/or long-term) and economic (direct and indirect costs). Adhering and remaining engaged in the daily drug treatment appear to be a challenge for the chronic patient. In this context, we propose as a first step, to describe the evolution of the wording around adherence. This evolution is underlying the paradigm shifts behind this concept and opens new areas for action. Based on this new angle of view that represents the patient’s adherence with its drug therapy, we detail the various factors influencing medication adherence categorized into five dimensions: illness, medication, demographic and socio-economic, patient and/or patient support and care system. The impact of these factors is described and explanatory models underlying explicited. Understanding of the issues underlying adherence to drug treatment will help consider further personalized interventions, multi-faceted response to the multiplicity of influencing factors.
