Nous rapportons les premiers résultats à 5 ans de recul de l’implant Pi2 en pyrocarbone. 42 implants chez 39 patients ont été suivis de manière prospective. L’âge moyen à la chirurgie était de 63 ans. La technique chirurgicale consistait en une trapèzectomie, avec trapèctomie partielle, capsuloplastie dorsale et ligamentoplastie pour éviter la luxation de l’implant. Au recul global de 62,4 mois, 97,6 % des patients étaient très satisfaits ou satisfaits ; aucun patient n’était insatisfait. L’indolence était toujours obtenue. Les mobilités et la force étaient augmentées et comparables au coté controlatéral. Le délai moyen de reprise des activités était de 76 jours (30 à 240). L’analyse radiographique retrouvait deux luxations et des remodelés minimes chez 29 % des patients sans conséquence clinique. L’implant est bien toléré à 5 ans de recul. Il permet de conserver la longueur du pouce, mais aussi une récupération précoce avec une augmentation de la force et de la mobilité.
This prospective study aims to reports the first review of Pi2 pyrocarbon implants with a 5-year follow-up. We included 42 cases in 39 patients. Mean patient age at surgery was 63 years. Operative The surgical technique consisted on trapeziectomy and minimal trapezoidectomy with dorsal capsuloplasty and a ligamentoplasty to avoid the luxation dislocation of the implant. Overall satisfaction ranged from very satisfied to satisfied for 97 % of the patients, and none were dissatisfied. Pain relief was excellent. Mobility range and strength were increased and comparable to the opposite side. Return to full activities was complete after 76 days on average. The radiographic review reported two implant dislocations and minimal bone remodeling was present in 29 % of patients, with no clinical significance. The Pi2 implant is well tolerated at 5 years. The major benefits are preservation of the length of the thumb, increasing its strength and mobility, and the short recovery period.
