La mise en oeuvre des caméras hybrides TEMP/TDM offre aux médecins nucléaires d'importantes évolutions dans leurs pratiques quotidiennes. Les indications en cardiologie sont connues depuis déjà plusieurs années. Rapidement cette méthode, couplant l'optimisation de la sensibilité, grâce à la correction d'atténuation, à la localisation par les logiciels de superposition, a été utilisée pour améliorer l'exploration scintigraphique. La contribution dans les indications ostéoarticulaires et oncologiques a déjà été évoquée récemment, nous envisagerons ici l'apport de cette technique pour identifier les foyers inflammatoires ou infectieux des organes profonds. La recherche de ces foyers est souvent difficile et l'amélioration du rapport signal sur bruit obtenue en TEMP/TDM est un outil précieux pour l'interprétateur. De même, la localisation précise offre au clinicien une orientation diagnostique importante. C'est notamment vrai en pathologie inflammatoire et dans les entéropathies exsudatives où seule la technique TEMP/TDM est efficace ; c'est également le cas pour l'identification des foyers infectieux vasculaires. Par ailleurs, la parfaite localisation de foyer ectopique à visée de repérage préopératoire se montre précieuse pour des patients fragiles en offrant à l'équipe chirurgicale d'importants renseignements.

The disposal of SPECT/CT hybrid cameras offers the nuclear physicians a great evolution in their daily practice. The indications in cardiology have been known for several years and the search for correcting the attenuation is at the origin of the technology. Rapidly, this method pairing the optimisation of the sensibility - due to the attenuation correction - and the localization, has been used to increase the performance of most of the scintigaphic exams. The contribution in the osteoarticular and oncological indications is already well accepted. The purpose of this paper is to point-out the contribution of this technology in the difficult task to identify the infectious and inflammatory seats in the deep organs. The improvement of the ratio of signal to noise achieved in SPECT/CT becomes a precious tool. Likewise the accurate localization offers the clinician a significant orientation for the diagnosis. Today, the SPECT/CT exams are the most efficient of all the imaging technologies, to detect, digestive inflammatory pathologies or exsudative enteropathies, as well as the identification of a vascular infectious seat. Moreover, the ectopic seat acute localization becomes a precious pre-operating tool for these fragile patients as it gives important data to the surgical team.
