Structure approach in rhinoplasty - 01/09/11

Over the years, rhinoplasty surgeons have moved away from excisional techniques and have focused on preservation of support structures and reorienting tissues. This article discusses the author's thoughts on reorienting tissues and the use of innovative structural grafting. Emphasis is placed on minimizing resection of tissues and the use of cartilage grafting to increase support and create favorable changes in contour. Methods used to stabilize the nasal base such as columellar struts and caudal extension grafts are discussed. New concepts in surgery of the middle vault and nasal tip also are covered.
