As many as 30% of adults with colorectal cancer have some family history of colorectal cancer, suggesting a degree of inherited risk. This article reviews the inherited high-risk colorectal cancer syndromes, with an emphasis on the role of gastrointestinal specialists in the management of these patients. The value of access to registries, genetic services, and experts in these rare syndromes is presented. The unique role of endoscopy for screening, surveillance, and management of these patients is stressed. Finally, this article addresses common familial risk in colorectal cancer, with an emphasis on the role of the endoscopist in managing patients and directing recommendations for effective colonoscopic screening and surveillance.
