Introduction. - Le traitement des tuberculomes cérébraux fait classiquement appel à l'association de la rifampicine, de l'isoniazide, du pyrazinamide et de l'éthambutol. La place des fluoroquinolones reste à définir.

Exégèse. - Nous rapportons le cas d'une femme âgée de 36 ans présentant un tuberculome cérébral. Une quadrithérapie antituberculeuse était débutée, mais en raison d'effets secondaires graves, l'isoniazide et la rifampicine ont été arrêtés au quatrième mois de traitement. Une bithérapie associant sparfloxacine et éthambutol a été débutée et poursuivie pendant 12 mois, ce qui a conduit à la disparition de la lésion cérébrale.

Conclusion. - Notre observation démontre que la sparfloxacine est une alternative pour le traitement des tuberculomes cérébraux en cas d'intolérance ou de résistance aux antituberculeux majeurs.

Introduction. - Treatment of cerebral tuberculomas demands the combination of rifampicin, isoniazid, pyrazinamide and ethambutol. The place of fluoroquinolones remains to be defined.

Exegesis. - A 36-year-old woman presented with a cerebral tuberculoma. Treatment with an antituberculous quadritherapy regimen was begun, but because of serious side effects, isoniazid and rifampicin weren withdrawn during the fourth month. Sparfloxacin and ethambutol bitherapy was then started and continued for 12 months, which led to the disappearance of the cerebral lesion.

Conclusion. - Our observation proves that sparfloxacin is an alternative for cerebral tuberculoma treatment in case of intolerance or resistance to first-line antituberculosis drugs.
