Intérêt du suivi thérapeutique des antirétroviraux chez le patient insuffisant rénal: Exemple du ténofovir - 29/11/08

Le dosage plasmatique des antirétroviraux (ARV) est particulièrement indiqué dans trois circonstances: l'imputabilité d'un échec virologique à une résistance virale, le contrôle de l'observance et l'adaptation posologique dans certaines situations physiopathologiques particulières telles que l'insuffisance rénale et/ou hépatocellulaire. Le ténofovir DF (Viréad®) est la première prodrogue du ténofovir (PMPA), analogue nucléotidique, a avoir obtenu l'AMM en 2002 pour le traitement de l'infection par le VIH des adultes sous multithérapie, en échec virologique.
Le ténofovir est majoritairement éliminé par le rein. Les paramètres pharmacocinétiques d'un patient insuffisant rénal recevant 1 comprimé de 300 mg par jour de Viréad® sont donc modifiés. L'aire sous la courbe (AUC) est multipliée par 15 et la clairance plasmatique totale du ténofovir (Cl/F) est divisée par 13. Deux adaptations posologiques ont été proposées.
Comme pour les inhibiteurs nucléosidiques de la transcriptase inverse (INTI), le dosage plasmatique du ténofovir chez un patient insuffisant rénal est indispensable. Afin d'optimiser ce suivi, une méthode analytique par chromatographie liquide haute performance (CLHP) avec détection UV, simple et relativement rapide, applicable en routine peut être utilisée.

Antiretroviral drug monitoring in plasma is important to prevent virus resistance, check drug compliance, and follow up patients with renal and/or hepatic failures. Since 2002, tenofovir DF (Viread®) is the first prodrug of tenofovir (PMPA), nucleotide analog used in combinaison with other antiretroviral drugs for the treatment of adults infected with the human immunodeficiency virus (HIV) who failed or are intolerant to nucleoside analog therapy or are not controlled by their current antiretroviral regimen. As tenofovir is extensivily and rapidely excreted in urine, the follow up of treated patients with renal failure is critical. The pharmacokinetics parameters from one HIV-infected patient with renal insufficiency, treated with 300 mg of Viread® were modified. AUC (area under the curve) was increased by a 15 fold and the total plasmatic clearance (C//F) was reduced by a 13 fold. Two dose adjustments were proposed.
Like nucleoside analogs (NRTIs), tenofovir monitoring in plasma of patients with renal failure is essential. In order to optimize this drug monitoring, a high-performance liquid chromatography assay with UV detection, simple, relatively rapid, can be easily performed in hospital laboratories.
