Résumé |
Introduction |
L’hématome rétropharyngé est relativement rare. On le rencontre généralement en traumatologie ou lors d’une complication de prise d’anticoagulants ou après dissection de l’aorte ascendante. Sa survenue chez l’hémophile est exceptionnelle.
Observation |
Un patient hémophile de 23 ans a présenté un volumineux hématome rétropharyngé et du plancher buccal, de survenue spontanée, avec dyspnée, dysphagie et dysphonie. L’évolution a été favorable après un traitement médical adapté.
Discussion |
L’hémophilie A est une maladie hémorragique due à un déficit en facteur VIII. Ses manifestations cliniques sont non spécifiques et variables en fonction de la sévérité de la maladie. Les hématomes cervicaux sont rares, mais graves car ils peuvent mettre en jeu le pronostic vital. Le traitement repose sur la transfusion des facteurs de coagulation déficitaires et sur l’éducation du patient et de son entourage.

Introduction |
The retropharyngeal hematoma is rare and usually due to trauma, anticoagulation therapy complication or ascending aortic dissection. This presentation is extremely rare in hemophiliac patients.
Observation |
A 23-year-old hemophiliac patient presented with a spontaneous onset large retropharyngeal hematoma extended to the floor of the mouth associated with dyspnea, dysphagia and dysphonia. The patient underwent adequate and successful medical treatment.
Discussion |
Hemophilia A is characterized by a deficit in factor VIII. Clinical symptoms are not specific and vary with the level of the intrinsic factor. Hematoma of the cervical region is a rare but potentially life-threatening event. The treatment requires transfusion of the specific factor and education of the patient and his relatives.
