La microphtalmie postérieure avec pli papillo-maculaire est une anomalie rare, affectant les deux yeux et qui habituellement ne s’accompagne pas d’autres anomalies oculaires ou générales. Elle se caractérise par un segment postérieur court avec un segment antérieur normal. Nous rapportons le cas d’un garçon de 3 ans et demi qui présenta une forte hypermétropie. L’examen du fond d’œil mit en évidence un pli papillo-maculaire bilatéral. L’échographie en mode A et B confirma la microphtalmie postérieure. À travers cette observation, sont rapportés les caractéristiques cliniques de cette anomalie congénitale rare, sa pathogénie et son mode de transmission.

Posterior microphthalmos with papillomacular fold is a rare disorder that involves both eyes without other ocular or systemic abnormalities. It is characterized by a short posterior segment with a normal anterior segment. We report the case of a 3.5-year-old boy with high hyperopia. The fundus examination showed bilateral papillomacular fold and ultrasonography confirmed the posterior microphthalmos. We comment on clinical characteristics, pathogenicity, and inheritance modes of this rare congenital disorder.
