Nous rapportons la première observation, à notre connaissance, de kératocône unilatéral en miroir chez des jumeaux monozygotes. L’évolution de la réfraction, l’examen clinique, les données vidéotopographiques et les courbes d’élévation orbscan des jumeaux ont été relevés et analysés afin de s’assurer du caractère unilatéral strict du kératocône chez chacun d’entre eux. Il existait chez chacun des jumeaux un kératocône unilatéral peu évolutif dans le temps, localisé à droite chez l’un et à gauche chez l’autre. Dans les limites actuelles des moyens diagnostiques, il semble qu’il s’agisse du premier cas de kératocône unilatéral en miroir rapporté chez des jumeaux homozygotes. Plusieurs cas de pathologies en miroir ont été décrits. Ils concernent essentiellement les appareils rénaux et dentaires. L’apparition de pathologies en miroir serait due à la survenue d’un facteur génétique ou environnemental au cours de l’acquisition de la symétrie du zygote, et cela avant sa division en deux embryons monozygotes.

We report the first observation to our knowledge of mirror unilateral keratoconus in monozygotic twins. The observation focuses on monozygotic twins, followed for 5 years. The progression of the refraction, the clinical examination, the video-topographic data, and the Orbscan elevation curves of the twins were collected and analyzed. For each twin, there was slightly progressive unilateral keratoconus during that time, located on the right for the first twin and on the left for the second one. Within the current limits of our diagnostic means, it appears to be the first case of mirror unilateral keratoconus in monozygotic twins. Several cases of mirror pathologies have been described, mainly concerning the renal and dental organs. The appearance of such mirror pathologies may be the consequence of the rise of a genetic or environmental factor during the acquisition of the symmetry of the zygote, occurring before its split into two monozygotic embryos.
