Surgical management of slightly or non-displaced corporal scaphoid fractures by retrograde percutaneous screw fixation through the anterior trapezium horn: A single-center retrospective French study of 33 patients between January 2015 and January 2019 - 11/06/23

Introduction |
Percutaneous screw fixation has recently gained popularity as an alternative to conservative treatment to avoid prolonged immobilization. The placement of a screw in the central axis of the scaphoid has been shown to be biomechanically superior to its eccentricity. Still, it poses difficulties in performing percutaneous screw fixation via both palmar and dorsal approaches.
Objective |
We describe a palmar percutaneous screwing of corporal fractures of the scaphoid by a simple palmar transtrapezial approach allowing an optimal centering of the screw.
Method |
We selected patients operated on by the same surgeon using the palmar transtrapezial approach between January 2015 and January 2019 based on the coding used for these fractures and the operative reports. In addition, pre- and postoperative data were collected from the patient's computer and paper records and by telephone contact with the patients.
Results |
Thirty-three patients were included. Percutaneous screw fixation of the scaphoid was performed under locoregional anesthesia in the operating room with one arm in the supine position on the arm table. No hyper-extension of the wrist was performed. The Kirchner guidewire passed through the anterior horn of the trapezium and then into the trapezium-scaphoid joint. A screw replaced it after satisfactory centering in the axis of the scaphoid. Management took place on average within 12 days after the trauma. 75.8% were A2 fractures, according to Herbert's classification. The average operating time was 16.63minutes, and in 91% of the cases, the patient was hospitalized for one day. The variation of the scapholunate angle on the preoperative profile radiographs with the angle defined by the axis of the scaphoid screw and the lunate postoperatively was on average 2.94°. One patient presented nonunion, and four showed an undersized screw with a screw overhang requiring revision surgery.
Conclusion |
The transtrapezial approach to fixation of acute scaphoid fractures facilitates precise percutaneous screw placement in the central axis of the scaphoid. A study of long-term complications, including the degenerative impact on the scaphotrapezial joint, is needed to assess the safety of passage through the anterior horn of the trapezium.

En tant qu’alternative au traitement conservateur, le vissage percutané a récemment gagné en popularité pour éviter une immobilisation prolongée. Le placement d’une vis dans l’axe central du scaphoïde s’est avéré supérieur à son excentration sur le plan biomécanique mais pose des difficultés à la réalisation du vissage percutané par voie palmaire comme dorsale.

Nous décrivons un vissage percutané palmaire de fractures corporéale du scaphoïde par une approche transtrapézienne palmaire simple permettant un centrage optimal de la vis.
Méthode |
Nous avons sélectionné les patients opérés par voie transtrapézienne palmaire par le même chirurgien entre Janvier 2015 et Janvier 2019 à partir du codage utilisé pour ces fractures et des comptes rendus opératoires. Le recueil des données pré- per- et post-opératoire a été obtenu grâce aux données collectées dans le dossier informatique et papier du patient et par contact téléphonique des patients.
Résultats |
Trente trois patients ont été inclus. Le vissage percutané du scaphoïde a été fait sous anesthésie locorégionale, au bloc opératoire avec un bras en position allongée sur la table à bras, en supination et aucune hyper-extension du poignet n’a été réalisée. La broche guide de Kirchner passait dans la corne antérieure du trapèze puis dans l’articulation trapézo-scaphoïdienne et était remplacée par une vis SBI® après centrage dans l’axe du scaphoïde satisfaisant. La prise en charge avait lieu en moyenne dans les 12jours après le traumatisme. 75,8 % étaient des fractures A2 de la classification de Herbert. La durée opératoire moyenne était de 16,63minutes et dans 91 % des cas, le patient était hospitalisé une journée. La variation de l’angle scapho-lunaire sur les radiographies de profil en pré opératoire avec l’angle défini par l’axe de la vis scaphoïdienne et le lunatum post chirurgie était en moyenne de 2,94°. Un patient a présenté une pseudarthrose et quatre patients ont présenté une vis de taille non adaptée avec un débord de la vis nécessitant une reprise chirurgicale.
Conclusion |
L’approche transtrapézienne pour la fixation des fractures aiguës du scaphoïde facilite le placement percutané précis d’une vis dans l’axe central du scaphoïde. Une étude des complications à long terme, et notamment de l’impact dégénératif sur l’articulation scapho-trapézienne est nécessaire pour évaluer l’innocuité du passage dans la corne antérieure du trapèze.
