Towards real-time monitoring of COVID-19 nosocomial clusters using SARS-CoV-2 genomes in a university hospital of the French Alps - 05/05/23

• | Our goal was to estimate whether near-real-time monitoring of nosocomial clusters in our establishment, the Grenoble Alpes University Hospital, was feasible by querying electronic health records (EHRs) combined with the calculation of genetic distances between sequenced samples of COVID-19 patients. |
• | An approach based on both EHR data interrogation and genomic data analysis is feasible and particularly interesting for prospective cluster management by an Infection Prevention and Control unit. It enables identification of potential nosocomial clusters in very close to real time with a significant time saving compared to classical epidemiological surveillance, and facilitates understanding and characterization of nosocomial clusters. |
• | This approach is fundamental because it affects cluster management in different clinical units. In addition to early cluster management, it prevents the infection of our patients by reinforcing suitable measures. Furthermore, restricting hospital visits makes more sense in the case of a cluster of infections by different lineages than in the case of a cluster with a single lineage as it suggests active viral circulation within a given unit. |

Objectives |
Experience of Nextstrain [[1], [2]] and its approach adapted to the local context encouraged us to carry out real-time monitoring of COVID-19 nosocomial clusters in our establishment, the Grenoble Alpes University Hospital.
Patients and methods, Results |
Through identification from electronic health records of nosocomial pathways and clusters and calculation of genetic distances from sequenced samples of COVID-19 patients, we were able to identify potential nosocomial clusters in very close to real time with a significant time saving compared to classical epidemiological surveillance, and to better understand and characterize nosocomial clusters.
Conclusion |
Through early detection and characterization of clusters, we may prevent infection of our patients by further implementing the appropriate measures.
