Dans le continuum de la spondyloarthrite axiale (SpAax), la SpAax non radiographique (SpAax–nr) correspond au stade caractérisé par l’absence de dommages structuraux établis dans les articulations sacro-iliaques. La naissance d’un concept selon lequel la SpAax aurait deux stades, le premier non radiographique et le second radiographique, a suscité diverses travaux visant à améliorer le diagnostic précoce et le traitement de cette maladie, mais a également donné lieu à de multiples controverses. Dans cet article, nous présentons plusieurs eujeux liés au concept de SpAax-nr, au diagnostic et au traitement, dans un contexte historique et à la lumière de données plus récentes.

Non-radiographic axial spondyloarthritis (nr-axSpA) is part of the spectrum or continuum of axSpA and is characterized by the absence of definite structural damage in the sacroiliac joints. The introduction of the concept of axSpA with two–non-radiographic and radiographic stages stimulated activities related to the improvement of early diagnosis and treatment of this disease, but also opened a number of controversial topics in the field. In this article, we are discussing several challenges related to the concept of nr-axSpA, the diagnosis, and the treatment in the historical context and in the light of the recent data.
