The Walk Hop program: A French innovative multicentric home cardiac tele-rehabilitation initiative - 31/12/22

Introduction |
While in-centre cardiac rehabilitation (CRC) has demonstrated benefits on morbidity and mortality, 65% of patients do not access it. The development of innovative programs in order to increase the possibilities of care is a public health issue.
Objective |
The “Walk Hop” home cardiac tele-rehabilitation (HBCT) program seeks to demonstrate its ability to safely and appropriately manage coronary patients usually absent from conventional programs.
Method |
In total, 3200 coronary patients with a RARE score below 4 will be recruited in 8 French cardiac rehabilitation centers. Duration=42months. Following an initial evaluation at the CRC, the HBCT solution (20 sessions) developed by Ensweet is proposed. This multidisciplinary platform consists of a secure portal and an application allowing the patient to communicate with the CRC (monitoring alerts). Patients will be equipped with a bicycle ergometer and a heart rate monitor. The patient application offers personalized physical rehabilitation sessions with muscle strengthening, aerobic activity and therapeutic education selected by the CRC and collects session data, program adherence (completion and compliance), symptoms and quality of life criteria as well as the mileage saved.
Results |
At 3months of study, 52 inclusions, male 80%, age m=55y, diabetes (13.2%), hypercholesterolemia (31.6%), high blood pressure (28.9%), tobacco (23%), no cardiac event during 1067 home sessions, 76.6% compliance (Fig. 1), less than 1 alert/patient (completeness and safety) and 593km saved/patient.
Conclusion |
This new form of home rehabilitation (remote monitoring and digital assistance) shows from the first months a safe alternative and well followed by each patient, versus rehabilitation in the center. This offer seems to meet the objectives of expanding the management of cardiac pathologies.
