Effects of modern hepatoprotector on condition of the liver indicators and central nervous systems of patients with alcoholic toxic hepatitis - 15/08/22

Aim |
To study the effects of the use of a complex hepatoprotector in acute alcohol poisoning complicated by toxic hepatitis.
Method |
The object of the study was 254 patients with alcoholic hepatitis who were treated at the toxicology department in 2015–2019. The study was carried out in two groups of patients. Group I–108 patients admitted in 2018–2019 who, in addition to traditional therapy, received a complex hepatoprotector “Remaxol” containing succinic acid, nicotinamide, riboflavin and ademetionine–400.0ml intravenously once a day starting from the moment of admission for 5 days. Group II (comparison group) - 146 patients who applied in 2015–2016, who received traditional therapy. We studied the level of ALT, AST, bilirubin, free ammonia, lactate was at admission and in dynamics on the 5th day. Assessment of the severity of intellectual disabilities was studied using the MMSE scale and the Reitan test on the 3rd and 5th days.
Results |
The level of blood alcohol in all groups was–2.1±0,9g/l, the indices of enzymes of group I were–ALT-184,2±41 u/l, AST–165,7±52 u/l, bilirubin–28,3±4.2mmol/L, free ammonia–66,2±9,5 mkmol/L, lactate 5,6±1,1mmol/L. In group II–Alt–177,9±63,3, AcT–159,5±55, bilirubin–24,2±3,7, ammonia–61,7±11, lactate 5,7±1,5. In dynamics we observed a decrease in ALT, AST, bilirubin, free ammonia, lactate by 2.1, 2,2, 1,4, 1,7, 2,4 times in relation from the initial data in group I. In the comparison group, the level of similar indicators was worse - only 1,3, 1,2, 1,1, 1,1, 0,9 times respectively. According to the MMSE scale, patients of group I on the 3rd day showed mild cognitive impairments–22±1,4 points, and on the 5th there was no cognitive deficit–28±1,6 points. In group II, the MMSE scale indices for all positions on the 3rd and 5th days were 1.5 and 1.4 times worse, respectively, than in group I. Reitan's test in patients of group I–118±12seconds, it was worse than in group I by 1.4 times.
Conclusion |
The use of a modern hepatoprotector effectively relieves cognitive disorders in patients with alcohol intoxication.
