Current management & future directions in post-liver transplant recurrence of viral hepatitis - 23/12/21

Advances in surgical technique, perioperative care and immunosuppression along with a better understanding of the pathology have dramatically improved the outcomes of liver transplantation (LT). Historically, post-LT recurrent viral hepatitis (rVH) were a major cause for graft loss. More recently with elucidation of viral pathogenesis, genomic analyses and the advent of potent antiviral medication, the results of LT for viral hepatitis (VH) parallel those for other indications. Early identification and a systematic approach is vital in the successful management of post-LT rVH. Striking a fine balance between suppressing immunity and maintaining optimal host anti-viral response remains the goal in these patients. An early initiation of appropriate antiviral therapy helps achieve viral control or clearance. This review summarises the current evidence on the management of post-LT rVH with a focus on its impact on graft and recipient outcomes.
