Nous proposons une ostéotomie cunéiforme fermée du radius distal avec l'objectif de supprimer la douleur et d'améliorer la fonction chez les patients présentant une arthrose post-traumatique de l'articulation radioscaphoïdienne par pseudarthrose de l'os scaphoïde. Cette intervention ne recherche pas spécifiquement la consolidation. Nous présentons une série prospective avec un suivi maximal de 16 ans.
De 1973 à 1990, 100 ostéotomies de fermeture de l'extrémité distale du radius étaient réalisées en première intention. L'intervention consistait en la résection sur la métaphyse d'un coin à base latérale de ≤ 0,5 cm sans modification de la longueur du radius ni modification de la fonction de l'articulation radio-ulnaire distale. Vingt patients étaient perdus de vue et trois avaient un suivi de moins de deux ans. La série présente correspond aux 77 patients (tous travailleurs manuels de sexe masculin) suivis au moins deux ans (moyenne 4 ans, extrêmes 2-16 ans). Les résultats étaient évalués avec une cotation spécifique pour la main scapho-pseudarthrosique (max 60 points) comportant six éléments pour la douleur, les résultats radiographiques, la mobilité du poignet, la force de préhension, la capacité de travail et la fonction du poignet.
À la dernière consultation de suivi, la cotation globale spécifique était de 45,66 points comparés à 19,98 avant l'intervention. Pour les douleurs, la cotation était de 8,51 points comparée à 3,19 points : 67 patients (86 %) n'avaient plus de douleurs ou des douleurs minimes lors d'un effort important. Une consolidation osseuse ou une fusion fibreuse sans signes d'arthrose était constatée chez 49 patients (64 %) et une arthrose post-traumatique moins sclérotique, considérée comme une amélioration, chez 11 patients (14 %). Tous les patients sauf deux ont repris leur travail.
L'ostéotomie cunéiforme permet de diminuer les douleurs et d'améliorer la fonction de la main en réduisant la pression appliquée sur les surfaces articulaires radiales et scaphoïdiennes. Elle a un effet positif en cas d'arthrose post-traumatique en modifiant la dynamique du poignet, et en offrant les mêmes effets bénéfiques que la styloïdectomie. La consolidation stable observée chez 27 patients à la dernière consultation était une constatation attribuée en toute probabilité au traitement ; elle n'était pas l'objectif recherché.
Management of painful pseudarthrosis of the carpal scaphoid bone with closed wedge osteotomy of the distal radius: a prospective study with long-term follow-up |
Purpose of the study |
We propose a different management scheme for painful post-traumatic osteoarthritis of the wrist resulting from pseudarthrosis of the carpal scaphoid bone. We report a prospective series with a maximum follow-up of 16 years.
Material and methods |
From 1973 to 1990, we performed 100 first-intention wedge osteotomies of the distal radius in patients with painful pseudarthrosis of the carpal scaphoid bone. Inclusion criteria were painful wrists with established pseudarthrosis. The operation consisted in the removal of a small lateral wedge (base ≤ 0.5 cm) without affecting the length of the radius nor the function of the distal radio-ulnar joint. Twenty patients were lost to follow-up and follow-up was less than two years for two. The present series corresponds to 77 patients (all male manual laborers) followed for at least two years. Mean follow-up was 4 years (range 216 years). To evaluate outcome, we developed a devoted scoring system, the Scaphoid Pseudarthosis Hand Score, (SPHS, max 60 points) with items for pain, range of motion, grasping strength, work capacity, wrist function and post-traumatic arthritis.
Results |
At last follow-up, the SPHS was 45.66 points versus 19.98 before surgery. The pain score was 8.51 versus 3.19: 67 patients (86%) were pain free or experienced mild pain for heavy work. Bone fusion or fibrous union was observed in 49 patients (64%) who were free of post-traumatic osteoarthritis; some improvement of which was observed in 11 other patients (14%) with less sclerotic bone. All patients but two resumed their occupational activities.
Discussion |
Wedge osteotomy relieves pain and improves hand function by reducing the pressure applied to the radial and scaphoid joint surfaces. It acts positively in the event of post-traumatic arthritis, altering the dynamics of the wrist and producing the beneficial effects of styloidectomy. Although it was not the target, sound union of the pseudarthrosis observed in 25 patients at last follow-up was attributed to the applied treatment.
