04-38 - A CORRELATIVE STUDY OF REFLECTED HIGH INTENSITY MOTION SIGNALS (RIMS) WITH STANDARD DOPPLER PARAMETERS IN THE EVALUATION OF GLOBAL LV FUNCTION IN CHILDREN - 09/04/08
Aim: Usefulness of Doppler Artifacts (DA) in the evaluation of LV function in adults was proved recently and was named as Reflected high intensity motion signals (RIMS). At the same time Tei index (myocardial performance index) is proved to be a very good indicator of global LV function. In the current study our aim was to correlate the values obtained by RIMS with standard Doppler parameters, and Tei index to establish the usefulness of RIMS in evaluating global LV function in pediatric age group.
Materials and Methods: 35 healthy volunteers of pediatric age group up to 18 years with a mean age 12 ± 4.7 were examined for standard Transmitral and LVOT flows and the values were correlated with initially recorded RIMS parameters. Transmitral (E, A, E/A ratio) and LVOT (LVOT vel, IVRT, IVCT & ET) flow values were obtained by standard cursor placement. RIMS parameters (Sa, Ea, Ea/Aa ratio RIMS ivct, RIMS et, RIMS ivrt) were registered by placing the sample volume in the pulmonary tissue 3-5 cm laterally to the lateral mitral valve annulus in the apical 4 chamber view. Tei index was calculated as the sum of IVCT and IVRT divided by ET, registered by both standard Dopplerographic and proposed RIMS method.
Results: An excellent direct positive correlation was found for the systolic (Sa and LVOT vel r = 0,83) and diastolic (E & Ea r = 0.93, A & Aa r = 0.8, E/A & Ea/Aa r = 0.83, IVRT & RIMS ivrt r = 0.62) components measured by both the methods. Tei index measured by the two methods also showed a moderate correlation (r = 0,54).
Conclusion: Reflected high intensity motion signals can (RIMS) can become an effective alternative method of Doppler study in the evaluation of LV function in children.
