Persons with symptoms that suggest a generalized neuromuscular disorder pose considerable diagnostic challenges for primary care physicians and specialists in electrodiagnostic medicine. This article discusses a conceptual framework on which to tailor the electrodiagnostic evaluation. The electrodiagnostic consultant must maintain a broad, inclusive differential diagnosis. Flexibility during testing is necessary to refine the list of diagnostic possibilities as information is collected. A clear understanding of what is normal provides the proper context for judging electrodiagnostic findings. Fortunately, many peripheral neuromuscular conditions manifest themselves in characteristic ways on electromyography (EMG) and nerve conduction testing, making them identifiable to the skilled consultant.
