Distal median motor latency and electromyography were historically the first electrophysiologic tests to be studied in patients with suspected carpal tunnel syndrome. Since that time, motor studies have been modified to increase their senstivity and thus, remain an essential component in the evaluation of carpal tunnel syndrome. Median CMAP latency, median-ulnar motor comparison, inching, amplitude and duration, and stimulation above and below the carpal ligament have been developed as tools for the motor evaluation of carpal tunnel syndrome. The decision to use needle electromyography as a diagnostic tool should be based on the patient’s history and physical exam
