Aspect TDM et IRM des radionécroses cérébrales chez 28 patients irradiés pour une tumeur ORL : corrélation de l'imagerie avec la radiothérapie - 19/03/08
Objectif. Décrire l'aspect en imagerie des radionécroses cérébrales chez les patients irradiés pour une tumeur ORL et corréler les résultats avec la radiothérapie.
Matériels et méthodes. Dans une étude rétrospective allant de 1986 à 1998, 1 201 patients porteurs de 981 tumeurs du nasopharynx et 220 de tumeurs naso-sinusales ont été irradiés, 28 ont présenté une radionécrose cérébrale. Tous ont eu une IRM et 18 un TDM. Le diagnostic était posé sur l'imagerie et confirmé sur l'évolution.
Résultats. L'incidence de la radionécrose cérébrale était de 2,33 %. Le temps de latence était compris entre 2 mois et 9 ans. Le scanner, normal dans 4 cas, retrouvait un oedème et/ou une prise de contraste focale. Chez tous les patients, l'IRM montrait un oedème et/ou une prise de contraste punctiforme ou gyriforme. Les localisations étaient uni ou bifocales : temporales (n = 18), frontales (n = 9), protubérantielles (n = 3), nerf optique (n = 2). Les doses d'irradiation ont été de 13 à 135 Gy dans les zones de nécrose. Dans deux cas, la nécrose était située en limite de champ. L'évolution était variable : rémission complète (n = 3) ou partielle (n = 1), poursuite évolutive (n = 11), atrophie séquellaire (n = 5), stable (n = 7).
Conclusion. L'IRM est l'examen le plus sensible pour le diagnostic et la surveillance. Le développement de nouvelles techniques de radiothérapie et d'imagerie devrait permettre de réduire l'incidence de cette lésion.
CT and MRI of 28 patients with cerebral radiation necrosis following radiotherapy for nasopharyngeal or ethmoidal carcinoma: correlation with the radiation technique |
Purpose. To describe and correlate with radiation therapy the occurrence of cerebral radiation necrosis in patients irradiated for nasopharyngeal or ethmoidal tumor.
Materials and Methods. From 1986 to 1998, 1 201 patients, 981 with nasopharyngeal tumors, and 220 with ethmoidal tumors were treated by radiotherapy. Twenty eight developed cerebral necrosis. MRI were performed in all patients and CT in 18 patients. Diagnosis was considered at imaging, and confirmed by follow-up.
Results. The incidence of cerebral radionecrosis was 2.33%. The time interval between treatment and necrosis ranged from 2 months to 9 years. CT showed edema and/or focal enhancement in all cases except for 4 patients with normal CT examinations. MRI showed edema and/or abnormal focal punctate or gyriform enhancement in all patients. Lesions were localized in the temporal lobe (n=18), frontal lobe (n=9), pons (n=3) and optic nerve (n=2). The doses related to the areas of necrosis ranged from 13 to 135Gy. In 2 cases necrosis was situated at the boundaries of the radiation field. Imaging follow-up showed complete (n=3) or incomplete remission (n=1), lesion progression (n=11), cerebral atrophy (n=5) and stability (n=7).
Conclusion. MRI is useful to diagnose cerebral necrosis. New technologies may reduce the incidence of this complication.
