Objective |
To record patient opinions and highlight conditions influencing patient satisfaction concerning pain control practices in Emergency Departments (ED).
Design and patients |
A 14-day multicenter prospective survey conducted in a French district, including ED adult patients reporting “severe”, “extreme” or “unbearable” pain on a verbal rating scale (VAS). Data collected included pain score and analgesia procedures.
Main outcome measures |
A questionnaire was used to assess satisfaction. Results were analyzed from the VAS score and the analgesia level. The chi-squared test was used to identify factors related to patient satisfaction.
Results |
Two thousand five questionnaires were analyzed for 5,753 eligible ED patients. Among 722 patients enduring severe pain (36%), 274 were satisfied by pain management (38%). Unsatisfied patients complained of excessive delay (41%) and analgesia inefficiency (44%). Factors correlated to patient satisfaction were analgesia level (83% with opioid use, 48% with intravenous level 1 analgesics, 34% without treatment and 32% with oral level 2 analgesics) and VAS final scoring procedure (44% vs 24%).
Conclusions |
Despite the high prevalence of severe pain in ED, most patients are not satisfied with pain control practices and complain of treatment delay or inefficiency. Initial and final pain scaling, intravenous treatment and opioid use should be encouraged to improve patient satisfaction concerning severe pain management in ED.
