L’implantation d’une assistance ventriculaire gauche à visée définitive est désormais une option thérapeutique pour certains patients en insuffisance cardiaque réfractaire non candidats à la transplantation. Nous rapportons ici l’observation d’un patient assisté depuis plus de quatre ans avec la pompe axiale INCOR de Berlin Heart. Cette observation montre la faisabilité d’une assistance au long cours avec une pompe à débit continu et d’une stratégie anti-thrombotique innovante reposant sur l’association d’héparine de bas poids moléculaire et d’anti-aggrégants plaquettaires.

Implantation of definitive left ventricular support is now a therapeutic option for certain patients in refractory heart failure who are not candidates for transplantation. Here we report the case of a patient assisted for more than 4 years with an INCOR axial pump from Berlin Heart. This case shows the feasibility of long term assistance with a continuous flow pump, and an innovative anti-thrombotic strategy relying on the combination of low molecular weight heparin with platelet anti-aggregants.
